Your challenge is to complete as many of the fall prevention tasks as possible in the month of September. Instructions for each challenge are listed below. Find your scorecard on the back page of this pamphlet and keep track of your points. Each task is worth a set amount of points. Some activities (like daily exercise) may be completed as many times as you like.

Falls are the leading cause of injury-related emergency department visits. This day is used to address this public health issue. NCOA's 2021 Falls Prevention Awareness Week campaign features a new Falls Free CheckUp, a digital screener where older adults can answer 12 simple questions to get their falls risk score and steps to reduce their risk. Need help using this tool? Stop by center from 10-11 am and we will be happy to assist you with your free screening.
